- Providers please contact NCF Diagnostics & DNA Technologies via phone: 1-352-375-5553 or email: support@ncfdna.com to order test kits, shipping bags and other supplies.
- The test request form will be prefilled with the provider information including the physician’s name, NPI Number, practice address, phone number, fax number and email address.
- The test kit with a request form, collection instructions and specimen collection tube will be shipped.
- Additionally, the FedEx/UPS envelopes with pre-paid return labels and account information are provided to ship the patient specimen to our laboratory for testing.
- All kits and return envelopes will be promptly packaged and shipped out to physicians, clinics and hospitals.
- Providers are encouraged to place multiple patient specimens and ship in a single FedEx/UPS envelope to easily track the shipment and save shipping costs.
- NCF Diagnostics & DNA Technologies will arrange pick up from nearby providers in the surrounding areas of Florida.
- Providers may also deliver patient specimens at our Laboratory.
- Providers can call FedEx/UPS to schedule pick-ups. Alternately, the provider can call our laboratory to arrange for FedEx or UPS to pick up specimens.
- NCF Diagnostics & DNA Technologies will work with providers to collect the patient specimen, receive quickly at the laboratory for processing and provide timely test results.
- The Patient Comes First! Every effort will be made to do everything for the patient.
